Chamber dental programs provide an added benefit and increase employee satisfaction.

 

Superior Dental Care (SDC)

The following plans are available exclusively to members of the Southern Ohio Chamber Alliance (SOCA) for groups with as few as 2 enrolled employees. These plans are guaranteed for 12 months when implemented by December 31, 2006. Employer groups with 20 eligible employees may fax a census of employees to SDC for a custom-designed plan.

oral exams, x-rays, cleanings, fluoride treatments for children, emergency treatment

Basic:

fillings, root canal therapy, oral surgery, extractions, repairs & recementation

Major:

crowns, onlays, bridges, dentures, sealants for children, periodontal gum treatment

All Dental Plans receive a complimentary vision plan through EyeMed Vision Care

 

All services must be rendered by a participating dentist to be eligible for coverage.
Please visit SDC's website www.superiordental.com for a directory of participating dentists and EyeMed discounts and locations. Pre-determination of benefits is necessary if services are for $250.00 or more and for periodontal services. Alternative benefits are based on the least expensive, professionally acceptable course of treatment. If the member and his/her dentist decide on a more expensive treatment, the additional cost will be the member's responsibility. All services are subject to the policies and procedures of SDC.

SMILERIDER™ Program

Superior Dental Care's SMILERIDER™ is a supplemental cosmetic rider that is offered to all Superior Dental Care plans at no additional cost. The cosmetic rider provides a 15% discount for elective cosmetic services such as teeth whitening, veneers, bonding and porcelain facings.  Online SMILERIDER™ Brochure (PDF).

Elective dental procedures are becoming increasingly popular; however, very often the costs associated with many of these procedures serve as a barrier. SDC’s SMILERIDER™ program helps to eliminate the barriers. The best part about this value-added discount is that it adds nothing to a group’s premium! This is due to special arrangements made with our participating dentists.

At a time when most dental carriers are implementing waiting periods, pre-existing conditions and other restrictions to their plans, SDC is pleased to be offering an enhanced dental plan at no cost to our members. Our goal is to keep you smiling for a lifetime!

EyeMed Vision Care Plus Plan

This program is offered to all SDC plan members.  You and your eligible family members will automatically receive discounts toward eye examinations, contact lenses and eyeglasses at every LensCrafters and Optique location world-wide and at any participating optometric provider.  To find a participating provider near you, please visit www.eyemedvisioncare.com or call toll-free 877/226-1115.

The vision plan will take effect at the same date of your dental plan's effective date and can be utilized for 24 months thereafter.  This will not replace any company-sponsored vision plans; it simply provides additional options and savings.

You will receive ID cards for this program and must present the ID card to receive benefits.  The card use is unlimited during the 24-month period for you and your eligible family members.

The EyeMed Vision Care Plus Plan also provides a 15% savings off the retail price of Lasik and PRK procedures at all U.S. Laser Network Centers.
